Download 2012 Founderes Cup Application Package (PDF). This PDF includes 1) Team Invitation Letter, 2) Rules, 3) Team Application Instructions, 4) Guest Player Application, and 5) Referee Plan. Team Application and Referee Applications are available for on-line. You may use the online Team Roster, fill in the Team Roster form by hand, or use your AYSO team roster. After downloading the 2012 Application Package, click on the start button below to submit a team application process.
